Forms Overview (Desktop)

Forms is the central place to build, manage, and review templated forms, such as daily hazard assessments or site inspections. Workers can fill these out on a repeating basis once they're assigned to a site, and every submission is tracked back to the worker and site it came from.

There are seven tabs in Forms: Submitted, Shared Forms, Library, Shared Links, AI Form Builder, Anonymous Submissions, and Archived.

Submitted

Lists every form that's been accessed and submitted across your company. Use Export to export the list based on your current filters, or Start Form to fill one out yourself. Filter by Status, Site, Form Title, Submitted By, or Submitted Date Range, and use Save Filter to reuse a filter combination later.

Opening a Completed or Pending Review form lets you sign it, review it (approve, disapprove, or leave comments), edit it, download it, or email it. Opening a Draft lets you delete it, view its details, share it, view it as a PDF, or sign and submit it.

Approve and Disapprove only appear if the form has Review Required enabled and you have Site Access or Full Access permissions.

A form always reflects the template as it was on the date it was copied, so later changes to the template won't affect forms already submitted.

Shared Forms

Shows forms that have been shared with you. From here, you can view, download, or email a form, and opening it gives the same sign, review, edit, download, and email options as the Submitted tab.

Library

Contains your form templates. Click Create Form to build a new one from scratch, or click a template's title to edit it. The Published column shows a check mark if the template is live and available, or an exclamation mark if it's still in progress.

Creating and editing form templates requires Full Access permissions with the Admin toggle enabled.

Shared Links

Lists links that have been shared with you, along with the associated Name and Site. From here you can generate a QR code for a link or delete it.

AI Form Builder

Upload an existing PDF form and Rosie, Salus's AI assistant, will read it and build a matching Salus form template automatically. Click Create Form, then Select PDF Forms and Send.

Note: Rosie does her best to recreate the form accurately, but you should always double-check the generated template before publishing it.

Anonymous Submissions

Holds forms submitted anonymously through a QR code with Anonymous Access enabled. These stay fully separate from your other submissions, and the submitter's identity is never captured.

Archived

Holds draft forms that have gone inactive, either archived manually or through your company's Auto-Archive Inactive Drafts setting. Click the recover icon in the Actions column to bring a form back.

Submitted Tab: Table Definitions

Title

Defaults to the title of the template it was created from, but can be renamed once a form is filled out.

Site

The site tagged on the form when the worker filled it out.

Asset

The asset the form references, if applicable. Shows blank or NA if no asset applies.

Status

Completed: submitted with no review required, or submitted and approved. Draft: accessed but not yet submitted. Pending Review: submitted and awaiting approval. Disapproved: submitted and rejected by a reviewer.

Tip: the Submitted filter option shows any form with a status of Completed, Pending Review, or Disapproved.

Created On / Created By

When the form was first started, and who started it.

Date

The date and time the form was submitted.

Frequently Asked Questions

Why can't I edit a form template?

Creating and editing templates in the Library requires Full Access permissions with the Admin toggle enabled. Check with your Company Admin if you need this.

Why don't I see Approve or Disapprove on a submitted form?

Either the form template doesn't have Review Required enabled, or your permission level is Own Access or Limited Access. Both need to be true for these options to appear.

If I update a form template, will already-submitted forms change too?

No. A submitted form always reflects the template as it was on the date it was filled out. Only new forms started after your changes will use the updated template.

Can I recover a form from the Archived tab?

Yes. Use the recover icon in the Actions column next to the form.

Should I double-check forms created with AI Form Builder before using them?

Yes. Rosie does her best to recreate the uploaded PDF accurately, but you should always review the generated template before publishing it.
