Anonymous forms
Overview
Anonymous forms let a worker submit a specific form without revealing their identity, and in some cases without even having a Salus profile. Use this when you want workers to feel free to report concerns, share feedback, or flag issues without worrying about being identified.
Warning: Before enabling anonymous submissions, keep these in mind:
- Anonymous submissions don't work with webhooks or auto emails.
- Anonymous submissions don't appear in any Salus reporting dashboards.
- Switching an existing template to Anonymous breaks any non-anonymous QR codes it was already added to.
- We recommend creating a new form template for anonymous submissions, rather than converting one you're already using.
Step-by-Step Instructions
Set a form template to Anonymous
- Click Forms in the left-hand navigation, then click the Library tab.
- Select the template you want to allow anonymous submissions for.
- In the form editor, click the Form Type dropdown on the right and select Anonymous.
- Click Save.
Create an anonymous QR code
- Click QR Codes in the left-hand navigation, then click + Create QR Code.
- Check the Anonymous Access checkbox in the bottom-left corner.
- Build out the rest of the QR code as usual. Only templates with Form Type set to Anonymous can be added once this checkbox is selected.
Once the QR code is created and posted, anyone can scan it and submit the form, whether they have a Salus profile or not, and their identity stays hidden.
View anonymous submissions
Anonymous submissions are always kept separate from your other submitted forms. Find them on your Forms screen, under the Anonymous Submissions tab.
What Happens Next
Once someone scans the QR code and submits, the form lands in the Anonymous Submissions tab with no identifying information attached. It won't trigger any webhooks or auto emails you may have set up, and it won't appear in your reporting dashboards.
Frequently Asked Questions
What happens if I convert an existing form template to Anonymous?
It stops appearing in the normal submit list on both Mobile and Desktop, and any non-anonymous QR codes it was part of will stop working. We recommend creating a new template instead of converting one already in use.
Can anonymous forms trigger webhooks or auto emails?
No, this isn't currently supported for anonymous submissions.
Will anonymous submissions show up in my reporting dashboards?
No, anonymous submissions aren't currently viewable in any Salus reporting dashboard. Find them in the Anonymous Submissions tab instead.
Can someone submit an anonymous form without scanning a QR code?
No, anonymous forms are only accessible by scanning their QR code.
