Anonymous forms allow your worker to submit specific SALUS forms without revealing their identity or, in some cases, without having a SALUS profile at all. This feature ensures workers can report concerns, provide feedback or share important information freely in a fully confidential way.
⚠️ Before you enable your forms for anonymous submissions please note the following:
- Anonymous form submissions are currently not compatible with webhooks or auto emails
- Forms submitted anonymously are not currently viewable in any of your SALUS reporting dashboards
- If you change a form template to anonymous it will cease to work in any "non anonymous" QR codes it was previously added to
- We recommend setting up a new form template for anonymous submissions rather than enabling an existing one
You can start equipping your workers with anonymous forms right away with two simple steps.
Step 1 - Form Templates
Before you begin, decide which form templates in your library should allow anonymous submissions. Here is how it's done.
⚠️ Note: Once your form template is set to "Anonymous" this form will no longer appear in the list of forms available to submit from either SALUS mobile or SALUS desktop.
Step 2 - Configure Your Anonymous Form QR Code
Anonymous forms are only available to be submitted by scanning a QR code. In order to make them accessible you now need to set up a QR code.
Once your QR code is created and put in place anyone can scan it and submit the form whether they have a SALUS profile or not and their identity will remain hidden.
Step 3 - View Anonymous Forms
Anonymous forms submitted in SALUS will always be kept separate from your other submitted forms. You can access them on your forms screen in the "Anonymous Submissions" tab.
