Start a form (desktop)

Overview

Forms let you complete and submit safety documentation in Salus. You can fill out a digital form on screen, or upload a completed PDF and attach it to the workers it applies to.

Use this when you need to record a safety check, inspection, or any other form your company has set up. This article covers the desktop workflow in Salus Desktop.

Start a form

  1. Click Forms in the main navigation. This opens the Submitted tab, which lists forms already submitted.
  2. Click Start Form, the blue button in the top right. The Create Form window opens.

Choose your form

The Create Form window opens with two dropdowns.

Form Title (required)

Selects which form you want to complete. The list shows the forms your company has set up.

Use it when: always. You must choose a form before you can continue.

Site

Links the form to a specific site. Whether this field is required depends on the form. Some forms are set to require a site, others are not.

Use it when: the form is tied to a job site. If the form requires a site, you cannot continue until one is selected.

  1. Select a form from the Form Title dropdown.
  2. If a Site is required, select one from the Site dropdown.

Note: If the form does not require a site, the Site dropdown can be left empty. The requirement is set on the form template, not chosen here.

Choose how to complete the form

The Create Form window has two buttons. Pick the one that matches how you want to complete the form.

Fill out a digital form

  1. Click Create Digital Form. The form opens on screen.
  2. Complete the form fields.
  3. Submit the form.

Upload a completed PDF

Use this when you already have a completed form saved as a PDF.

  1. Click Submit Upload. A file picker opens.
  2. Select your file. Only PDF files are accepted.
  3. After the PDF is added, a Search Site Workers dropdown appears. Select the workers this form applies to. You can scroll the list or type to filter it, which is faster for larger worker lists.
  4. Submit the form.

Note: Only PDF files can be uploaded. Other file types will not be accepted by the file picker.

What happens next

Once you submit, the form is recorded in Salus and appears in the Submitted tab under Forms. If you selected a site, the form is filed under that site. Uploaded PDFs are also filed under the workers you selected in the Search Site Workers field.

Frequently Asked Questions

What is the difference between Create Digital Form and Submit Upload?

Create Digital Form opens the form on screen so you can fill it in and submit it. Submit Upload lets you attach a completed form that you already have as a PDF.

Why does the Site field appear required on one form but not another?

The site requirement is set on each form when its template is built. Some forms require a site and some do not, so the behavior changes depending on the form you select.

Can I upload a Word document or photo instead of a PDF?

No. The file picker accepts PDF files only. Convert your file to a PDF before uploading.

The Search Site Workers field did not appear. Why?

The Search Site Workers dropdown only appears after you add a PDF through Submit Upload. If you are filling out a digital form instead, you will not see this field.

How do I find a worker in a long list?

The Search Site Workers field is both a dropdown and a search box. Start typing a name to filter the list, which is quicker than scrolling for larger companies.

Can I fill out a form on my phone?

Yes. Most forms are completed in the field on Salus Mobile. This article covers the desktop steps. See the Salus Mobile version for phone instructions.
