The Media tab lets you add fields that capture or display images, files, links, and signatures. These options are especially useful for field workers using mobile devices who need to document work, attach evidence, or sign off on tasks.
Note: Selecting any field type adds it to your form immediately. A Field Name is required before the field is complete, so name it right away.
Field Types
Single Media Attachment
Allows the user to attach one file to the form, such as a photo, video, or PDF. Field workers can take a picture directly from their phone or upload an existing file.
Use it when: you want one piece of supporting evidence, such as a photo of completed work or a single inspection file.
Multiple Media Attachment
Lets users attach more than one file within the same field. Supports photos, videos, and PDFs, uploaded or taken from a mobile device.
Use it when: you need several supporting images or documents for a single step or checklist item.
Web Link
Adds a clickable link that opens an external webpage.
Use it when: you want users to access a resource such as a company website, manufacturer manual, or online training reference.
Embedded Documents
Lets you place a video, PDF, or image directly inside the form. You can select an existing file from your Salus Documents or upload a new one. Users can view the document right within the form itself, making it easy to reference safety materials while completing tasks.
Use it when: you want users to watch a short safety video, review a PDF, or view an image directly in the form. Ideal for JHAs, training steps, or any workflow that needs quick in-context guidance.
Note: Salus supports the following file types as form attachments: AVI, JPEG, JPG, MOV, MP4, PDF, and PNG. Size limits apply based on file type: videos up to 50 MB, PDFs up to 5 MB, and images with no size restriction. For larger files, consider uploading to the Salus Documents module and referencing them from there instead.
Signature
Adds a digital signature field where one person can sign directly on screen using a mouse, stylus, or touch input. If multiple signatures are needed, add a separate signature field for each signer.
Use it when: you require confirmation, verification, or sign-off from a worker or supervisor.
What Happens Next
As soon as you select a field type, it's added to your form. Name it and set any available options, and it's ready to use. On mobile, Single and Multiple Media Attachment fields let workers capture photos or video directly from their device camera instead of only uploading existing files.
Frequently Asked Questions
What's the difference between Embedded Documents and Document (in the Selects tab)?
Embedded Documents displays the file's content directly inside the form for viewing, like a video or PDF someone should watch or read. Document (in the Selects tab) links to a file the user can open, without embedding it inline.
What file types and size limits apply to attachments?
Salus supports AVI, JPEG, JPG, MOV, MP4, PDF, and PNG. Videos are limited to 50 MB, PDFs to 5 MB, and images have no size limit. For anything larger, upload it to the Documents module and reference it from there.
Can more than one person sign the same Signature field?
No. Each Signature field captures one signature. If you need multiple sign-offs, add a separate Signature field for each signer.
Can workers take a photo directly from their phone, or do they need to upload an existing file?
Both. Single and Multiple Media Attachment fields let workers either take a new photo or video on the spot, or upload a file already saved on their device.
