Create and Send a New Action
Overview
Action items let you flag issues, assign corrective actions, and track follow-up directly in Salus. You can create an action from two places: the Actions module, or from within a submitted form by clicking the flag icon next to a specific field.
Both methods use the same fields and produce the same result. The difference is context: creating from a form ties the action to a specific form question, while creating from the Actions module is a standalone action not linked to a form.
Method 1: Create from the Actions module
- Click Actions in the left navigation menu.
- Click Create Action Item in the top right corner.
The Create Action Item dialog opens. Fill in the fields that apply.
Site
The site this action is associated with. Select from the dropdown.
Optional.
Assign To
The worker responsible for completing this action.
Optional.
Type
The category of this action item. Select from your company's active action types. If no types are available, ask your admin to create them under Actions › Types.
Optional.
Due Date
The date by which this action should be completed.
Optional.
Attachment (paperclip icon)
Attach a file or image to support this action item.
Optional.
Priority
The urgency level of this action: Low, Medium, or High.
Optional.
Describe Action
A free-text description of what needs to be done and why.
Optional.
- Click Send Action.
The action item is created and appears in the Submitted list immediately. If you assigned it to a worker, they will be notified.
Method 2: Create from a submitted form
Use this method when reviewing a submitted form and you want to flag a specific field for follow-up. The action is linked to that form question.
- Click Forms in the left navigation menu.
- Open the submitted form you want to review.
- Find the field you want to flag and click the flag icon next to it. The tooltip reads Add Corrective Action.
The New Action Item dialog opens, titled with the name of the field you flagged. The site is pre-filled from the form. Fill in the remaining fields: Type, Assign To, Due Date, Attachment, Priority, and Describe Action.
Tip: When creating from a form, Priority defaults to Medium. Adjust it before sending if the issue requires a different urgency level.
- Click Send Action.
The action item is created and linked to the form. The form's Action Required counter in the top right updates to reflect the new active action. The action also appears in the Actions Submitted list.
Frequently Asked Questions
The Type dropdown is empty. What should I do?
No active action types have been created yet. Ask your admin to create them under Actions › Types. Only types set to Active will appear in the dropdown.
Will the assigned worker be notified?
Yes. When you assign an action to a worker and click Send Action, Salus notifies them so they know there is a task requiring their attention.
What is the difference between creating from Actions and creating from a form?
Creating from the Actions module produces a standalone action not linked to any form. Creating from a form links the action to a specific field on that form, and the form's Action Required counter tracks it. The form method also pre-fills the site from the form.
Can I edit an action after sending it?
Yes. Open the action from the Actions Submitted list and make your changes.
I don't see the flag icon on the form. What's wrong?
Flag icons only appear on forms that have been configured to allow corrective actions on individual fields. If the form doesn't show flag icons, it may not be configured for this feature. Contact your Salus admin.
