How do I use the AI Form Builder to create a form?

Note: Before you begin, please ensure your existing form is in the PDF format (At this time we only support forms in PDF format) and note where on your laptop/desktop computer the PDF is saved.

  1. From the AI Form Builder page, click the ‘Create AI Form’ button.
  2. From the pop-up click the ‘Upload PDF Form’ and select the PDF you have previously saved locally on your laptop/desktop computer.
  3. Once the PDF has been selected you are now ready to submit the file to our AI Form Builder. To begin this process click ‘Send'
  4. After clicking ‘Send’ you’ll now see the name of the form, as well as the time you submitted the form to the AI form Builder.
  5. The final piece of information concerns the current status of this build process. The length and complexity of the PDF submitted will determine how quickly the form will be ready for your review, at which point the status will switch from ‘Running’ to ‘Complete’. Please note the time can vary from just a few minutes to over 20 minutes. However, you do not need to remain on this page while this process is ‘Running’. You may safely navigate away from this page and return at a later time.
  6. Once your form has been built the status will now show as ‘Complete’. Clicking on a completed form will allow you to select ‘Use Result’. Clicking this option will open the generated form in our form builder.
  7. We encourage you to review the generated form to ensure elements on the form look and work as expected. Please feel free to manually make any adjustments to the generated form that you see fit.

Please ensure you give the form a title and select a form type, as well as reviewing any other applicable settings for the form. Once complete click ‘Create’ to publish the form and make it available to your company's SALUS users.
