Forms Overview (Mobile)
Digital forms are available any time from Salus Mobile, so you can complete them on site as work happens. In-progress forms save automatically as drafts, and every submission is kept in your history so you can refer back to it later. You can also share a form with other workers so they can add to it and sign under their own name, saving them the time of filling it out from scratch.
Typical forms include daily hazard assessments, site hazard reports, and COVID self-assessments.
Interface
The Forms screen has four tabs: New, Drafts, Submitted, and Shared.
New
Lists every form available to your company, grouped by category. Tap a form to open it, fill it out, and select the applicable site. If you already have drafts saved for a form, a badge shows how many you have in progress. Use the filter icon next to the search bar to narrow the list down.
Drafts
Shows forms you've started but haven't submitted, ordered with the most recent at the top. From a draft, you can:
- Share, so other workers can view and add to what you've filled in, and optionally submit it themselves.
- Action, to create a corrective action linked to the form, which appears in the assigned person's Inbox.
- Signatures, to sign the form.
- Rename, to relabel the draft for easier reference.
- Delete, to remove the draft.
- Preview, to see the form as it currently looks.
- Email, to send the form to an entered email address.
Submitted
Shows all forms submitted for the sites you're assigned to, most recent first. Use the filter icon to narrow results by Site, Form Type, Status, or Date Range, or use the Email icon to select forms and send them to an entered email address. Opening a form gives you options to Email, Review, and Preview it. If you have sufficient permissions, you'll also see options to Sign, Approve, or Disapprove.
Shared
Shows forms that have been shared with you, whether they're in draft or submitted status. From here you can sign a submitted form directly, but if a shared form is still a draft, you'll need to go to the Drafts tab to edit it.
Permissions
Salus roles are fully customizable, so what a worker can do with forms depends on the Permission Level assigned to their role, not a fixed role name. There are four Permission Levels:
- Full Access — can fill out and submit forms, and review, approve, or disapprove any form with Review Required enabled, company-wide.
- Site Access — can fill out and submit forms, and review, approve, or disapprove forms with Review Required enabled for their assigned sites.
- Own Access — can fill out and submit forms, but can't review, approve, or disapprove them. Visibility is limited to forms they've personally signed or submitted.
- Limited Access — has no access to company functionalities, including forms.
Note: Approve and Disapprove require both conditions to be true: the form template must have Review Required turned on, and the reviewer must have Site Access or Full Access. If Review Required is off, there's no approval step regardless of the reviewer's permissions.
A separate Temporary Worker role exists for short-term or guest users, such as visitors or auditors, who only need to complete specific assigned forms or orientations without visibility into broader company data. For an ongoing contractor or seasonal worker, it's better to create a custom role (for example, "Contractor") with Own Access, which lets them complete their assigned forms and orientations while limiting them to their own information.
Frequently Asked Questions
Why don't I see Approve or Disapprove options on a form?
Either the form doesn't have Review Required enabled, or your Permission Level is Own Access or Limited Access. Both the form setting and your permissions need to be in place for these options to appear.
What's the difference between a role and a Permission Level?
A role is a custom name your company creates (for example, "Foreman" or "Contractor"). Every role is assigned one of four Permission Levels, Full Access, Site Access, Own Access, or Limited Access, and that Permission Level is what actually determines what a worker can do with forms.
Can I edit a shared form that's still a draft?
Not from the Shared tab. Go to Drafts to make edits. The Shared tab only lets you sign forms that have already been submitted.
Should I use the Temporary Worker role for a seasonal worker?
No. Temporary Worker is meant for short-term guests like visitors or auditors. For a seasonal or ongoing contractor, create a custom role with Own Access instead.
What does the badge on a form in the New tab mean?
It shows how many drafts you currently have in progress for that form.
